CERTIFIED BIRTH CERTIFICATE
SOCIAL SECURITY CARD
And
CURRENT SCHOOL ENROLLMENT FORM
(With West Virginia Physical Address)
-OR-
CURRENT SCHOOL ENROLLMENT FORM
(If P.O. Box number listed only)
2 PROOFS OF WEST VIRGINIA RESIDENCY (See Below)
 

Then You MUST Bring:

CERTIFIED BIRTH CERTIFICATE
SOCIAL SECURITY CARD
(No School Enrollment Form Required)
And
2 PROOFS OF WEST VIRGINIA RESIDENCY (See Below)

Documents reflecting any name changes since birth (certified marriage certificate, divorce certificate or a court order)
 

Proof of Residency Requirements include:
(Only 2 of the Following Documents Required)

  • If the youth is living at home, the parent or legal guardian must provide proof of residency documentation
  • Tax records indicating a WV physical address
  • Current utility bills indicating WV physical address (not a P.O. Box number)
  • Residential rental and lease agreements for a WV residence
  • Mortgage documents for a WV residence
  • A W-2 form not more than 18 months old with a WV physical address
  • A current concealed weapons permit with a WV physical address
  • A motor vehicle registration card in the name of the applicant issued by the Division with a WV physical address
  • WV Voter's Registration Card
  • Proof of receipt of WV Public Assistance
  • WV Homestead tax exemption
